The Hormone Symptom Tracker

Symptoms are easy to forget and easy to dismiss on a good day. This simple tracker turns I feel off into a clear pattern you can bring to your provider. Two weeks of tracking is often more useful than any single lab drawn on a single day.

How to use it

  1. Each evening, take two minutes to rate the categories below from 1 (low or none) to 5 (high or strong).
  2. Note your cycle day if you are still cycling. Day 1 is the first day of your period.
  3. Track for at least two weeks, ideally a full cycle.
  4. Bring the completed tracker, and the questions at the end, to your next appointment.

There are no good or bad scores. You are simply gathering information so your labs and your lived experience can be read together.
